OUR SERVICES 

Home Health Services

 

The Medicare certified VNA Home Health Services provide skilled services as ordered by the physician.  The Visiting Nurse Association develops a care plan in conjunction with the patient’s physician to address the acute needs and help patients and families become empowered to care for and manage their acute conditions or chronic illnesses.

 

Home Health services are covered 100% by Medicare and Medicaid and covered under most insurance plans. 

 

Skilled Services include:

 

Skilled Nursing Occupational Therapy
Physical Therapy Speech Therapy
Social Work

Home Health Aides

 

Each third party payer including Medicare and Medicaid has qualifications for coverage of home health services.  Refer to your benefits handbook for coverage details or call the Visiting Nurse Association and ask to speak to a Patient Advocate.

 

How do I get the Visiting Nurse Association to provide home health care to me or my loved one?

 

Home Health Services require a Physicians order.  If the person requiring home health is not in a health care facility, the Visiting Nurse Association can assist in obtaining physician orders.  Simply call 513-345-8000 and ask to speak with a Patient Advocate.

 

If the individual is in a health care facility – hospital, skilled nursing facility, rehabilitation unit etc., the patient will be assigned to a discharge planner.  The facility discharge planner is required to ask if the patient has a choice of home care providers. Simply “ASK FOR THE VNA”, the discharge planner will provide us with the physician orders and we will visit the patient the day after they come home from the facility.

  


Personal Services

 

The Visiting Nurse Association also offers non-skilled, non-medical services including personal care services and homemaking.  These services are offered through the Council on Aging programs which includes Elderly Services program (ESP), Passport (PP) and Northern Kentucky Area Development District.  Tell your case manager that you would like your services delivered by the Visiting Nurse Association.

Light Housekeeping Laundry

Meal Planning

Ironing
Meal Preparation

Changing bed linens

Shopping/Errands

 

           

Respite

We provide companionship and supervision for the individual who require 24-hour supervision.  This service allows the family caregivers to leave the home for a short period of time and feels confident that their loved one is safe at home.

 


Private Services Plus

 

The Visiting Nurse Association also provides private services.  Services you want, when you want them.   Private Services do not require a physicians order or authorization from a third party payer.  Services are delivered through a direct contract between the VNA and the recipient making the request.  Services can be customized to meet your needs up to 24 hours a day 7 days a week.

Our personnel drive client’s car or escort in a taxi.

 

Case Management

 We provide the Coordination of services and personal needs.  A VNA nurse will be the link between patient and family and/or trust officer to make sure that all of the health, home and personal needs are addressed.

 

Assist with setting appointments and ensuring that transportation etc. is arranged Assist with household bills
Assist with coordination of service organizations for household repairs Assessment and oversight of medical condition and needs
Oversight and coordination of medication management Coordination of all health care providers

 

Lifeline (Emergency Response System)

In case of emergency or need of assistance, a simple press of the button on the supplied necklace or bracelet will dispatch to a family/friend/neighbor or emergency medical team.

 

MD-2 Medication Dispenser

 The medication dispenser machine automatically dispenses the right medications at the right time with a reminder alert.
