Our focus is on the patient and the family rather than the illness, with a goal to improve their quality of life within their own home setting, where they are most comfortable, at ease, and fulfilled. VNA Hospice Care provides compassionate care, meeting the medical, emotional, and spiritual needs of individuals and their families. We assist individuals in finding peace, meaning, and comfort while embracing this journey, even helping them take advantage of opportunities to complete goals, heal relationships and make memories.

Hospice Team

Hospice Care by the VNA uses a team approach to providing health care. The team, including the patient and their family, will develop a plan of treatment designed to meet a patients goals of “quality of life”. A patient’s plan will include emotional, spiritual, and social aspects of coping as well as treatment for areas of discomfort. Hospice services are provided by members of the Hospice Care Interdisciplinary Team which includes:

  • Patient’s Physician and Hospice Physician
  • Social Worker
  • Registered Nurses
  • Chaplain
  • Nurse Aides
  • Bereavement Counselors
  • Volunteers

The Hospice Care team is available for our patients 24 hours a day, 7 days a week, 365 days a year.
