Home Health Services
The VNA's Home Health Services are designed to help you get better, regain your independence, and become as self-sufficient as possible.

Home health includes a wide range of skilled services you receive in your home. In general, the services are provided on a part-time, or intermittent, basis. Our nurses and therapists have equal or better training and skills as their counterparts in a hospital or nursing home. But receiving care at home is far less expensive and much more convenient than in those environments.

The VNA is a Medicare/Medicaid certified home health provider. We're dedicated to earning your trust by providing you with the highest level of service. Plus, a VNA nurse is always available by phone 24-hour a day, 365 days a year.
When should you receive home health services?
  • When you have just been released from the hospital.
  • When you have a chronic disease that has worsened.
  • When you've had a recent acute illness, injury or surgery.
  • When you have new medications.
  • When you have mobility and/or balance issues.
  • How does home health help?
  • It assists you in recovering faster.
  • It can prevent unnecessary returns to the hospital or emergency room.
  • It provides you with in-home education, and also for family caregivers.
  • It can allow you to remain in your home, as long as possible.
  • It keeps your physician better informed.
  • How is eligibility determined?
    Home Health Services are 100% covered by Medicare and Medicaid. Most private insurance plans offer coverage too. Not sure about coverage? We will do an in-home assessment to assist you in determining if you qualify. Call us at (513) 345-8000.
    What services are included?
    A Home Health care plan may include one or more of the following services:
    Skilled Nursing
    Our nurses begin by providing a full assessment of all of a patient's body systems. Your VNA nurse will also assess the home setting, including a complete inventory of all of your medications and supplements. VNA nurses are experts in providing treatment and education to patients and their families to address individualized needs. In addition, your VNA nurse can provide wound care, disease management, and medication education, as well as coordinate care among other care providers and physicians.
    Physical Therapy
    Physical therapists help restore movement following an accident or illness. In the process, physical therapy can also reduce pain and prevent disability.
    Occupational Therapy
    Occupational therapists help restore the ability to "function" following an accident or illness, including eating, dressing, bathing, meal preparation and housekeeping, all to help the patient toward greater self-sufficiency.
    Speech Therapy
    Speech therapists help with problems with speech and language, as well as problems with hearing.
    Social Work
    Health problems can be devastating. The VNA has social workers available to help with financial, social and emotional concerns of the patient and family.
    Home Health Aides
    Home health aides work under the direction of our nursing staff and provide services such as administering oral medications, help with prescribed exercises, helping patients with bathing and dressing, and with housekeeping.
    How long will I receive Home Health Services?
    The home health plan of care will be developed with input from the physician and patient. Services can continue until all goals are met.
